Felony Bond Jumping Requests:

OBA Attorney Jeff Eulberg has drafted a generic form to use when  requesting your local DA to file Felony Bond Jumping Charges.    The link below is  a Printable PDF.

Death Certificates:

We have attached link below for a Printable PDF which outlines the required documentation for a Bondsman to request a Death Certificate.  You will find the specific Applicant section referencing Bail Bondsman at the bottom of page 2.  We are told you would fill out a death certificate application on their website and provide the required documentation listed in this attachment – located on page 3.
